What do I need to sign up for the Patient Portal?


You will need an email address. Don't have your own email account? We recommend signing up for a free account through Gmail, Microsoft, or your Internet Service Provider.

If you register in person, you will also be asked to choose a secret question (such as "What year were you married?" or "What are the last 4 digits of your SSN?") and a corresponding answer to verify your identity.

If you register online, you will need to provide your Social Security number as your Patient Match Identifier.

How do I sign up?


You can sign up for the Patient Portal by speaking with our registration staff the next time you check in for an appointment, or by clicking the register online button above. You will need to provide an email address in both cases.

Can I register my child for the Patient Portal?


Yes! The next time your child has an appointment with us, use your email address to start their Patient Portal registration process. When you receive the registration email and click the invitation link, the website will ask "Are you (child's name)?" Choose the option for "I am managing this account." Continue the registration process from there.

Any children over the age of 18 will be automatically removed from their parents' accounts, as they will no longer be considered minors.

Can I still access the old Patient Portal?


LMC's previous Patient Portal is no longer available. To obtain your health data from the previous patient portal, you will need to call Lavaca Medical Center to make a records request.
